For those who completed the insurance application for their organization, please log-in to request any of the documents listed below. Once you have logged-in, under the “Welcome” tab in the “Members Dashboard”, you will find various Wizards to help guide you through the document request process.
If the individual responsible for insurance at your organization has previously submitted written approval allowing you to request documents on your organization’s behalf please visit our contact page to submit your request via email.
If you are not the primary insurance contact and have not been granted written approval by the primary insurance contact we are unable service your request. Please contact your organization’s primary insurance contact to either submit the request on your behalf and/or submit written approval for you to request documents moving forward.

If you have recently submitted payment for an application and did not choose to pay the expedite fee, please allow 5-7 business days for processing. (Special verbiage may take longer).
